Supported Living Services
Our core service. Individually tailored support delivered in a person's own home — leased in their name, furnished to their taste, run on their schedule.

Care Connection Services partners with adults with developmental disabilities to live independently in their own homes — with the right support, at the right moment, always on their own terms.

What we provide
No two service plans look alike. We start with what someone already does well, listen to what they want next, and build a team around that — reviewed continually with the individual, their family and their Regional Center service coordinator.
Our core service. Individually tailored support delivered in a person's own home — leased in their name, furnished to their taste, run on their schedule.

Flexible, respectful assistance with routines, cooking, housekeeping and personal care — scaled to exactly what each person wants help with.
Appointment scheduling, medication support, and clear communication with medical teams and families.
Budgeting, bill paying, shopping and tenancy support that builds real, lasting financial confidence.
Getting to work, class, appointments and the places that make a neighborhood feel like home.
Patient, strengths-based coaching toward the goals each person names — work, friendships, hobbies and belonging.

Our approach
We were founded by direct support professionals who believe the people they serve deserve more say in their own lives. Over the past seven years, that belief has guided a small, close-knit practice — one that supports a limited number of individuals at a time so every relationship gets the attention it deserves.
The person decides. We plan around their choices, not our convenience.
Consistent staff who know someone's story, humor and routines.
Independence built gradually, and celebrated at every step.
Their home, their rules, their records — protected without exception.
How it works
A steady, transparent process — so families and referral partners always know what happens next.
A service coordinator, family member or individual reaches out. We meet — no pressure, no paperwork yet.
We spend real time learning routines, preferences, strengths and the goals that matter most.
Together with the IPP team we write a plan that names supports, hours and outcomes in plain language.
We hand-select support professionals for compatibility, then introduce them gradually.
Ongoing check-ins, 24/7 on-call backup, and a plan that flexes as independence grows.

Belonging
Work, friendships, faith communities, volunteering, the corner coffee shop where they know your order. We build support around the connections that make a life feel full.
“For the first time my brother is making his own decisions about his day, and the team behind him treats every one of them as though it matters. That changed our whole family.”
